“I look exhausted even when I’m not.” We hear that sentence at AMMA more than almost any other. Usually the person saying it is pointing at the same spot, the soft hollow that runs from the inner corner of the eye down toward the cheek, and usually they’ve already been told online that filler is the answer.
Sometimes it is. Often it isn’t. The under eye is the thinnest, least forgiving skin on the face, and it punishes the wrong treatment in a way the cheeks and jawline never do. So before you book anything, it helps to understand what is actually causing the shadow you see in the mirror, because the fix for a hollow is not the fix for thin, crepey skin, and neither one fixes pigment.

Three different problems that look identical in the mirror
Stand in bathroom lighting and every tired under eye looks the same. Up close, in clinic lighting, they separate quickly into three categories, and most people have some blend of all three.
- Volume loss. The fat pad that used to sit under your eye has slid or shrunk, so light falls into a groove and casts a shadow. Tilt your chin up toward a window and if the shadow lightens dramatically, this is a big part of your picture.
- Skin quality. The skin itself has thinned and lost collagen, so it looks crepey, finely lined, and slightly translucent. Blue tones from the vessels underneath start showing through. This is very common in your late 30s and beyond, and years of Miami sun accelerate it.
- Pigment. True melanin darkness, often genetic, often more pronounced in medium and deeper skin tones. It does not cast a shadow, it is a color change, and it does not respond to volume at all.
The reason this matters commercially as well as clinically: filler only addresses the first one. If you put filler under an eye whose real issue is thin skin or pigment, you get a puffier version of the same tired look, and you paid for it.
What EZ-Gel PRF actually is
PRF stands for platelet rich fibrin. We draw a small amount of your blood, spin it in a centrifuge with no additives, and separate out the layer dense in platelets, fibrin, and white blood cells. EZ-Gel takes that a step further with a controlled heating and cooling cycle that turns the liquid into a soft, honey-like gel made entirely from your own plasma.
That gel does two things at once. It gives a small amount of immediate softening in the hollow, and it releases growth factors over the following weeks that recruit your own fibroblasts to build new collagen. So the early result is subtle and the good result shows up later, usually somewhere between weeks six and twelve, as the skin itself gets thicker and less translucent.
There is nothing synthetic in it. Nothing to dissolve, nothing to migrate, no hyaluronic acid pulling water into an area that is already prone to morning puffiness. For patients whose main complaint is “my under eye skin looks thin and crinkly,” this is usually where we start. The honest tradeoff: it is a series, not a single visit. Most people need three sessions spaced about four weeks apart, and results build gradually rather than appearing the next morning. If you want an instant change before an event next week, PRF is the wrong tool.
When filler is genuinely the better answer
We are not anti-filler. For a well-defined, structural hollow in someone with good skin thickness and no chronic morning swelling, a conservative amount of hyaluronic acid dermal filler can look excellent, and it looks excellent the same day.
What we insist on being clear about: injecting the tear trough is an off-label use of HA fillers in the United States, and it is one of the higher risk areas on the face because of the vessels that run through it. It also behaves unpredictably. HA is hydrophilic, so it draws water, and in a humid climate with salty food and poor sleep, a result that looked perfect in September can look puffy by November. Placed too superficially it can throw a bluish cast called the Tyndall effect. Filler in this area can also persist far longer than the marketing suggests.
All of that is manageable in experienced hands, and it is reversible, dissolving runs $300 at AMMA. But it is a real conversation to have before you sit in the chair, not after. Frequently, the patient who came in asking for tear trough filler leaves having had filler placed in the upper cheek instead, supporting the area from below without ever injecting the delicate skin itself.

We look at your cheek support, since a flat midface makes any under eye look worse no matter what we put in the hollow.
Then we usually recommend less than you came in asking for. Starting with a PRF series and reassessing at week twelve costs less than a syringe of filler over three sessions and it is fully reversible by simply not continuing. If you still want structural volume after that, the filler goes in on top of better skin, which is exactly the order you want.

Frequently Asked Questions
Does EZ-Gel PRF hurt?
Most patients rate it a 3 or 4 out of 10. We numb the area with topical anesthetic first, and the gel itself contains no lidocaine, so you may feel pressure as it is placed. The blood draw bothers people more than the injections do.
How long does under eye PRF last?
Because PRF works by stimulating your own collagen rather than adding a substance, results after a full series typically hold for about 12 to 18 months before a maintenance session is worthwhile. Your own aging continues underneath it, so think of it as a reset rather than a permanent fix.
Can PRF fix dark circles?
Partly. If your darkness comes from thin skin showing the vessels beneath, thickening that skin genuinely lightens it. If the darkness is melanin pigment, PRF will not change the color much, and a pigment focused approach with medical skincare is a better use of your money.
What is the downtime?
Expect mild swelling for 24 to 48 hours and a real possibility of bruising, since the under eye bruises easily. Most people go back to work the next day, though we suggest scheduling at least two weeks before anything photographed.
Is under eye filler safe?
In experienced hands with conservative technique it is generally safe, but it is an off-label use of HA filler and the area carries a higher vascular risk than most of the face.
