Sweating Through Your Shirt in Miami? What Botox for Hyperhidrosis Actually Does

There is a specific kind of Miami math that a lot of our patients do without thinking about it. Gray shirts are out. Silk is out. You keep a change of clothes in the car, you find a reason to keep your jacket on at dinner, and you do the quiet calculation about whether you can raise your arm to hail a taxi. If that sounds like your life, you are not just someone who runs warm. You may have hyperhidrosis, and it is one of the most treatable things we see.

Botox is the treatment nobody expects, because most people file it under forehead lines. The same molecule that softens an eleven line also shuts down the signal that tells a sweat gland to fire. For underarms, it is FDA approved for exactly this purpose, and the results last far longer than they do on your face.

Sweating that much is a medical condition, not a personality trait

Primary focal hyperhidrosis affects roughly 3 percent of people. It usually shows up in the teens or early twenties, it runs in families, and it tends to hit specific zones: underarms, palms, soles, sometimes the scalp and hairline. The giveaway is that it happens regardless of temperature or effort. You are sweating through a shirt in an air conditioned conference room at 9 in the morning, and nothing about the situation explains it.

That is different from sweating because it is 91 degrees with 80 percent humidity, which is just September in Miami. It is also different from secondary hyperhidrosis, where sweating is a symptom of something else. Worth a visit to your primary care doctor before any cosmetic treatment if your sweating started suddenly in adulthood, wakes you up at night, happens on only one side of the body, or comes with fever, weight loss, or a racing heart. Those patterns point to a cause that needs treating, not masking.

What Botox actually does to a sweat gland

Your sweat glands do not decide anything on their own. They wait for a chemical messenger called acetylcholine, released by small nerve endings in the skin. Botulinum toxin blocks the release of that messenger. No signal, no sweat, in the small area around each injection point.

Nothing is destroyed, and nothing is permanent. The nerve endings gradually regenerate, the signal comes back, and the sweating returns. That is why this is a repeat treatment rather than a cure. It is also why it is so safe: the effect wears off on its own, and it stays where we put it.

What the appointment is actually like

Faster and easier than most people brace themselves for. We map the treatment area, sometimes with a starch iodine test that turns actively sweating skin a dark purple so we can see the borders rather than guess at them. Then we mark a grid, usually injection points about 1 to 2 centimeters apart.

The injections themselves are shallow, just under the surface, with a very fine needle. Underarms are genuinely easy, most patients rate them a 2 out of 10, and we ice beforehand. The whole thing takes about 15 to 20 minutes. You go back to work. There is no downtime, no bandage, and no reason anyone would know.

Timing runs a little differently than cosmetic Botox. You will usually notice dryness around day 2 to 4, and the full effect settles in at about two weeks. We ask patients to skip deodorant and heavy workouts for 24 hours afterward, similar to our standard Botox aftercare rules.

Units, areas, and what it costs

Neurotoxin at AMMA is $14 per unit, flat, for Botox, Dysport, and Daxxify. Sweat treatment needs more units than a forehead does, because we are covering surface area rather than a single muscle. The FDA studied dose for underarms is 50 units per side. Other areas are treated off label and are sized to the patient.

Hands, feet, scalp, and the areas people ask about quietly

Palms. This is the one that changes daily life the most. Handshakes, paperwork, phone screens, steering wheels. It works well, and it is the most uncomfortable area to inject, because your palms have a very high density of nerve endings. We use ice and topical numbing, and we go slowly. A small number of patients notice mild temporary weakness in fine pinch grip for a few weeks, which matters if you are a pianist or a surgeon, and we talk through that before we start.

Scalp and hairline. Very popular with our patients who blow dry their hair and watch it collapse on the walk to the car. Treating the hairline keeps a blowout intact for days rather than minutes.

Under the breasts, the groin, the back. People are often embarrassed to ask. Please ask. These are ordinary requests and we treat them routinely.

One combination worth planning: if you are also doing laser hair removal on your underarms, we schedule the two on separate visits rather than the same appointment, usually a week or two apart, so the skin is calm for each.

Who should wait, and what we screen for

We do not treat during pregnancy or breastfeeding. Neuromuscular conditions such as myasthenia gravis, Lambert Eaton syndrome, or ALS rule it out. Active infection or a rash in the treatment area means we reschedule. A known allergy to botulinum toxin or to human albumin is a hard no.

Beyond that, we talk about what you have already tried. Clinical strength antiperspirants with aluminum chloride, applied at night to dry skin rather than in the morning shower, genuinely help some people and cost almost nothing. If those have failed you, which is usually why people end up in our chair, Botox is the next reasonable step. Frequently Asked Questions

How long does Botox for sweating last?

Underarms typically stay dry for 4 to 6 months, and a fair number of patients get longer. That is noticeably more durable than cosmetic Botox on the face, which usually runs 3 to 4 months. Palms and soles tend to land in the 4 to 6 month range as well.

Will I just sweat more somewhere else?

Mild compensatory sweating elsewhere is reported occasionally, and it is usually subtle enough that patients do not ask us to do anything about it. It is a much bigger concern with surgical nerve procedures than with a localized injection, because we are only quieting a small patch of skin and the rest of your cooling system is untouched.

Does it hurt?

Underarms, barely. It is a series of quick pinpricks with a fine needle, and we ice first. Palms and soles are a different conversation, more sensitive because of nerve density, and we use topical numbing and take our time. Is this covered by insurance?

Some medical plans do cover botulinum toxin for diagnosed primary axillary hyperhidrosis, typically after you have documented that prescription antiperspirants failed. AMMA does not bill insurance. Can I shave, wear deodorant, and work out normally afterward?

Shave the day before rather than the day of. Skip deodorant and hard workouts for the first 24 hours. After that, everything goes back to normal, and most people find they need much less deodorant, or none.

September is a sensible time to start.
